If you are planning long term care, an informal meeting in the comfort of your own home is the best environment in which to talk through your care needs, perhaps in the company of a friend or member of your family. We will be happy to visit at your convenience. During this visit we will draw up a Needs Assessment and Care Plan in conjunction with you so that we have a full picture of your care needs, routine, and lifestyle. We will also conduct a risk assessment in order to help ensure your safety and that of the carers.

When we have a clear idea of your requirements we will choose one or more carers for you and we will then telephone you to discuss our choice. Your appointed carer will telephone you several days before arrival to introduce themselves to you.

Long term care usually involves a team of two carers working in rotation for one or two weeks at a time. Each will hand over to the next and we do our best to ensure there is never a gap in care and that continuity is maintained. Some carers, usually those from South Africa, Australia or New Zealand, are able to work for several weeks at a time with the same client.

At Convivium Care we keep in regular contact to make sure that all is going well and that you are completely satisfied.

Convivium Care does everything in its power to ensure continuity. Some carers have been regularly providing care to the same client for several years. At Convivium Care we do not change carers for our own convenience: if a client and his or her carers are happy together, we keep them together as long as both parties wish.
